Run BHI in-house with AI workflows
Cynnvi lets your existing care team run general BHI end to end, while AI reduces the documentation, time-capture, and coding work that normally eats up a coordinator's day.
How Cynnvi runs BHI, month to month
Identify patients
Cynnvi surfaces patients with a qualifying behavioral health condition already being treated by the practice, so staff know who's eligible for BHI each month.
Track the care plan
Care plan revisions and monthly assessment notes are logged as part of the ongoing BHI service, so the record stays current between visits.
Extended Workflow Automation add-on: Automate Care Tasks can send validated rating-scale questionnaires (like the PHQ-9 or GAD-7) by SMS between visits, and route any reply that needs staff into the same worklist.
Capture clinical staff time
Every minute of qualifying time is captured automatically from real EHR activity and traced back to the specific note, order, or referral behind it.
Extended Workflow Automation add-on: AI Receptionist adds inbound patient calls to the same worklist, so the staff follow-up they generate still gets captured and counted toward BHI.
Draft documentation
As soon as a care-management call or session ends, the AI Scribe drafts the clinical note. Staff confirm it, then it's written back to the chart.
Suggest the right code
Cynnvi recommends 99484 or G0323 based on who performed the qualifying time, clinical staff directed by the billing practitioner, or the clinical psychologist/social worker personally.
Prepare the monthly billing record
Approved time, documentation, and consent are assembled into a monthly billing record with the suggested code, ready for staff sign-off.
Who qualifies
- Patient has a behavioral health condition (or conditions) being treated by the billing practitioner, such as depression, anxiety, or substance use disorder
- At least 20 minutes of clinical staff time per calendar month directed by the billing practitioner (99484), or furnished personally by a clinical psychologist or clinical social worker (G0323)
- Documented patient consent to the BHI service
Codes at a glance

| G0570 | General BHI add-on for patients also receiving APCM | Parallels 99484's requirements, billed alongside an APCM code | ~$58 | Details → |
Approximate CY2026 Medicare Physician Fee Schedule national non-facility payment benchmark for non-QP clinicians, before geographic, sequestration, and claim-specific adjustments. What's required to bill
- Documented patient consent for the BHI service
- Only one general BHI code (99484 or G0323) billed per patient per month, based on who performed the time
- Care plan revision, systematic assessment and monitoring using validated tools where appropriate, and brief interventions
- Continuity of care and coordination with the treating practitioner
Documentation checklist
- Consent to the BHI service
- Time log for clinical staff (99484) or the clinical psychologist/social worker (G0323)
- Rating scale or monitoring results where used
- Care plan revisions based on the monthly assessment
Built for audit-ready compliance, with a human always in the loop
- Every AI-drafted note, task, and time entry is reviewed and approved by clinical staff before it counts toward billing, nothing is submitted automatically.
- Time capture traces back to the specific chart activity (note, order, or referral) behind it, producing an audit trail instead of a self-reported estimate.
- Staff attest to the accuracy of captured time and documentation before it's included in the monthly billing record.
- A signed Business Associate Agreement (BAA) is included, covering how protected health information is handled.
Common billing mistakes
- Billing both 99484 and G0323 for the same patient in the same month
- Billing 99484 or G0323 without reaching the 20-minute threshold
- Missing documented consent specific to the behavioral health service
Why clinics run BHI with Cynnvi, not a vendor
EHR-embedded workflow
Cynnvi opens from the patient's chart in the EHR your clinic already uses. No separate portal, no new login.
Clinic-operated, not outsourced
Your own staff run BHI with Cynnvi's help. The program isn't handed off to a third-party vendor.
No per-patient fee
One flat monthly price per clinic covers the program, regardless of how many patients are enrolled.
FAQ
Is this a BHI platform we manage ourselves, or an outsourced vendor?
Cynnvi is software your own clinical staff use, not an outsourced BHI vendor. Your team delivers care, reviews AI-drafted work, and stays the clinical decision-maker throughout.
Do we need a separate login or portal?
No. Cynnvi opens from inside the patient's chart in the EHR you already use, so staff don't need to switch between systems to run BHI.
Is there a per-patient fee?
No. Cynnvi is priced per clinic per month, not per enrolled patient, so growing your BHI panel doesn't change the price.
How does time capture work, and can we trust it for billing?
Time is captured automatically from real EHR activity (chart views, orders, referrals) and call duration, with every entry traceable to the specific action behind it. Staff review and approve captured time before it's used in the monthly billing record.
Does the AI replace our behavioral health staff?
No. AI drafts documentation and suggests the billing code, but staff review and approve everything before it's finalized. The goal is less write-up work for your existing team, not fewer staff.
Which EHRs does this work with?
Cynnvi supports Athenahealth, Aprima, DrChrono, eClinicalWorks, Elation, MDland, NextGen Enterprise, Office Ally EHR 24/7, Practice Fusion, and TouchWorks. See our integrations page for details on your EHR.
What counts toward the 20 minutes for general BHI (99484)?
Clinical staff time directed by the billing practitioner spent on care plan revision, systematic assessment and monitoring, brief interventions, and continuity of care for the patient's behavioral health condition.
When is G0323 billed instead of 99484?
G0323 is billed when a clinical psychologist or clinical social worker personally furnishes the 20 minutes of general BHI time, rather than clinical staff directed by the billing practitioner (99484).
Can BHI be billed alongside Advanced Primary Care Management (APCM)?
Not with the standalone codes (99484, G0323). Starting in 2026, CMS added G0570, an add-on code specifically for furnishing general BHI to a patient also receiving APCM in the same month, use that instead of the standalone codes.
Does Cynnvi support the Psychiatric Collaborative Care Model (CoCM)?
No. Cynnvi currently supports general BHI (99484/G0323), not CoCM (99492, 99493, 99494, G2214).
